“They’re too smart to have ADHD” is a common, understandable, and incorrect assumption. Intelligence and attention regulation are different systems, and research specifically testing this question found that a high IQ does not shield anyone from genuine ADHD.
What the research actually found
A study looking directly at intellectually gifted children with a confirmed ADHD diagnosis set out to test whether high measured intelligence changed the underlying cognitive picture. It did not. The children in the study showed real attention and executive function difficulties consistent with ADHD, despite excellent general cognitive ability. Why intelligence delays diagnosis instead of preventing it
What a high IQ actually changes is not whether ADHD is present, but how long it stays hidden. Strong cognitive ability gives a child more room to compensate: reasoning through a problem that should have required sustained planning, cramming effectively enough to pass a test despite chronic disorganisation, talking through a task rather than following the steps that were actually taught. That compensation can hold up for years, particularly through school structures built around exactly the kind of tasks a bright mind can push through despite an underlying attention difficulty.
It tends to break down later: when university, a demanding job, or independent adult life removes the scaffolding that made compensation possible, and the underlying difficulty that was always there becomes visible for the first time.

The comparison that actually matters
Attention difficulty in a gifted person is easy to miss because it is usually judged against the wrong baseline. If someone with an IQ well above average scores in the ordinary range on a measure of attention or organisation, that can look like nothing is wrong, until the comparison shifts to what that same person’s own cognitive ability would otherwise predict. Against their own potential, an average result on attention is not a null finding, it is a real gap. Screening built around population averages routinely misses exactly this pattern.
Why the two get mistaken for each other
Some surface behaviours overlap enough to confuse the picture in both directions. A wandering mind during a task that feels unchallenging, restlessness, and visible disinterest in repetitive work can describe either a gifted child who is genuinely underchallenged, or a child with ADHD whose attention struggles regardless of how interesting the material is. The distinguishing question is whether the difficulty is specific to boring tasks, which points toward underchallenge rather than a disorder, or shows up across tasks the person is genuinely engaged by too, which points toward ADHD. A single bored afternoon settles nothing either way; the pattern across both dull and engaging tasks is what actually distinguishes them.
Why some groups get diagnosed later than others
Masking, actively or unconsciously hiding symptoms to meet expectations, is not distributed evenly. Gifted girls in particular have been observed to develop more complete and longer-lasting masking than gifted boys, which is one documented reason their ADHD diagnosis often arrives roughly a decade later. The underlying difficulty does not wait for masking to fail; it is present the entire time, just invisible to anyone judging only by outward performance.
When to seek help
Speak to a doctor for a proper ADHD assessment if attention, organisation or follow-through difficulties are present across both interesting and uninteresting tasks and have been there since childhood, regardless of how strong academic performance has been. Good grades or a high measured IQ are not evidence against ADHD; they are, at most, evidence that compensation has been working so far.
